If you live in Yolo County and your Medi-Cal is Partnership HealthPlan, you can see an Advantage clinician in person at 1430 Blue Oaks Blvd, Suite 120, Roseville, or on video from home. We are in network, and you do not need a referral.
Woodland. Davis. West Sacramento. Winters. Yolo is close to Sacramento, so people mix up the plans. If the card says Partnership, you can see us. If it says Kaiser, it is not Partnership.

If your card says Partnership HealthPlan, you can see us. We are in network, and you do not need a referral. Contact us directly.
Covered visits are billed to the plan through Carelon. You do not pay us for a covered visit. We never ask for a credit card, and Medi-Cal members are never charged a cancellation fee.
We take Partnership HealthPlan Medi-Cal only. If your card shows a different Medi-Cal plan, including Kaiser Medi-Cal, or you need county specialty mental health, we will tell you on the free consult.

Look at the name on the card. Partnership. Or Kaiser. Those are not interchangeable in Yolo. If you are not sure, say so on the call. Bring the Partnership ID and the Medi-Cal BIC if you have them.

West Sacramento sits across the river from Sacramento. The county is still Yolo. If your Medi-Cal is Partnership, living in West Sacramento does not keep you out. Davis, Woodland, and Winters are the same rule.
West Sacramento, Davis, Woodland, and Winters can use the Roseville office if they would rather sit in a room. 1430 Blue Oaks Blvd, Suite 120. Fair Oaks is 8146 Greenback Lane, Suite 103F, Fair Oaks, CA 95628. Video is still available. Same clinicians either way. Addresses and maps are on locations.

My card says Kaiser. Can I still use this?
No. If the card says Kaiser, that is not Partnership. We cannot take Kaiser Medi-Cal. Ask on the consult if the name on the card is unclear.
Will you take my credit card?
No. Covered Partnership visits are billed to Partnership/Carelon. We do not keep a card on file and we do not charge you for a covered session. Medi-Cal members are never charged a cancellation fee.
I live in West Sacramento. Is that Sacramento County Medi-Cal?
West Sacramento is Yolo County. Coverage still depends on the plan on your card. Partnership: ask about video. Sacramento County Medi-Cal or Kaiser: we cannot take it.
